Job description
RaisingHealth in Brooklyn seeks a Community Health Partner (CHP) to educate, perform case management, and conduct outreach across health, social service, and education programs. The CHP will help families access community services, navigate benefits, and promote healthy behaviors.
The role requires bilingual Spanish or Chinese with English, two years of CHP-like experience, and strong communication. In-office, Tue–Sat 10am–6pm, Brooklyn office with competitive benefits.
