Job description
NYU Langone Hospital - Brooklyn seeks a Community Health Worker to join an interdisciplinary team serving patients with complex medical and behavioral health needs. The role focuses on engagement, education, and access to care across hospital and community settings.
Qualified candidates will have a High School Diploma/GED, at least 1 year in health services, and bilingual abilities (English/Spanish or other languages). The position is full-time with day and evening hours in Sunset Park, Brooklyn.
