Job description
Humana seeks a Bilingual Field Care Coordinator to support members in the Louisville, KY area. In this community-based role, you will conduct in-home and virtual assessments in English and Spanish, identifying medical, behavioral, and social needs. You will create and manage person-centered care plans, coordinate services with providers and community partners, and support care transitions to reduce hospitalizations. Using Humana’s digital tools, you will document encounters, close gaps in care, and educate members and families on benefits and resources, helping improve health outcomes and access to quality, culturally competent care.
