Job description
Humana is seeking a Bilingual Field Care Coordinator to support members in Louisville, KY through in-home and community-based visits. In this role, you’ll assess health and social needs, create personalized care plans, and coordinate medical, behavioral, and community services. You’ll use your bilingual skills to educate members on benefits and preventive care, address social determinants of health, and improve outcomes.
