Job description
Elara Caring is seeking a Home Health RN Case Manager to serve patients in Boston Proper, including Chinatown. You will manage a caseload, provide skilled nursing visits, and coordinate with physicians to ensure quality, patient-centered care.
You will assess needs, develop care plans, teach patients and caregivers, and travel to patients’ homes with a valid driver’s license. A strong clinical foundation and independence are essential.
