Job description
Care Partner Position
As a Care Partner in our Green House home, you are required to have a Texas nursing assistant certification (C.N.A) and you will provide person-centered care to Elders in a warm, home-like environment.
You'll assist with daily living activities, help maintain a clean and nurturing household, and promote independence and dignity for each Elder.
Responsibilities
Provide assistance with daily living needs such as bathing, dressing, grooming, toileting, and mobility
Prepare and serve meals according to Elder preferences; clean up kitchen and dining areas
Perform light housekeeping, laundry, and home maintenance tasks
Monitor and report changes in health, mood, or behavior to the nursing team
Encourage Elder engagement, independence, and decision-making
Work and communicate effectively as part of the self-managed work team.
Participate in care plan discussions and team meetings
Maintain accurate electronic documentation of care provided
Follow all safety, infection control, and confidentiality policies
Build meaningful relationships with Elders and team members in a supportive, self-managed work team
Qualifications
Certified Nursing Assistant (CNA) – active, unrestricted in the State of Texas
CPR and Food Handler Certification (can be obtained after hire)
Ability to lift up to 100 lbs. and perform physical care tasks
Compassionate, dependable, and team-oriented attitude
Excellent communication and observation skills
Training Provided
All Care Partners receive Green House model training and continuing education to support person-directed care and teamwork.
