Job description
One Medical is seeking a licensed RN in CA to join our Senior Health Care Team in an in-office, 40-hour/week role in Strawberry Village with possible onboarding in SF or Phoenix. You will conduct nursing visits (in-person/video/remote) and lead care coordination for senior patients across offices.
Ideal candidates have 2+ years nursing experience, exposure to care management or population health, and the ability to collaborate with providers and health navigators to close care gaps.
