Job Summary (Physical Therapist - Long-Term Care Facility, Vacaville, CA):
- Provide physical therapy services to residents in a long-term care facility
- Assess, plan, and implement individualized treatment programs
- Work with interdisciplinary team to maximize patient mobility, function, and independence
- Document patient progress and maintain accurate records
- Educate patients, families, and staff on therapy goals and techniques
- Full-time, day shifts; ASAP start
