About the Role
As a Case Manager, you'll coordinate services, complete documentation, and communicate with clients, families, providers, and county teams while monitoring client support plans. This role starts in-office and moves to fully remote after 90 days, or after 30 days for staff who complete orientation early. Your caseload is backed by a dedicated QA team and paraprofessional staff who support documentation and quality review, so you're not managing compliance alone. Your day will involve organizing your caseload, problem-solving client needs, responding to emails and calls, completing assessments and reports, and working with your support team to meet each client's needs and regulatory requirements. You'll also complete client visits and team meetings in person as needed, while managing your caseload independently.
Job Duties:
Coordinate services and supports for a caseload of approximately 40-50 clients with QA and paraprofessional support
Develop and monitor person-centered support plans, goals, and outcomes

Respond to client and team communications and address emerging needs
Maintain accurate records and complete quality assurance requirements
Follow Minnesota DHS regulations and company policies and procedures
Manage your schedule and caseload independently while meeting productivity and documentation expectations
