Job description
Hartford Healthcare seeks a Campus Care Hybrid Psychiatric APP to provide compassionate psychiatric assessment, treatment, and follow‑up for diverse patient populations in a hybrid on‑site/telehealth model. In collaboration with psychiatrists, nurses, and campus care teams, you will perform mental health evaluations, manage medications, develop evidence‑based treatment plans, and coordinate care across settings. You’ll document in the EMR, support crisis intervention, and participate in quality initiatives while benefiting from professional development and advanced clinical resources.
