AuthoraCare Collective is currently seeking a PRN CAP Case Manager (Social Worker) for our Burlington, Greensboro or High Point campus. This is a hybrid position.
Manage the over-all plan of care for a minimum case load of CAP clients as determined by the Manager of CAP programs. Assess and identify patient/caregiver problems, establish goals, and interventions. Monthly client contact and quarterly in-person visits for assessment of ongoing needs, interventions, and service utilization.
Requirements
Education/Qualifications:
Graduate of an accredited School of Social Work
One year of experience in social work case management required
Bachelor's degree in social work required
Two years of experience preferred
Case management accreditation or certification preferred by agency.
Other: Valid State-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
Delivery of Client Service Coordination
• Directs and coordinates all services provided through CAP to patients in accordance with the patient's individualized care plan.
• Utilizes case management process in planning and prioritizing patient care needs, including assessment and identification of patient/caregiver problems; identification
and implementation of interventions; and evaluation of outcomes.
• Partners with the assigned RN case manager to complete annual recertification process.
• Establishes relationship with patient/caregiver to adequately assess ongoing needs.
• Recognizes influence of culture, ethics, and beliefs on decision making.
• Makes home visits in accordance with NC Medicaid requirements.
• Offer telephonic support to CAP clients as needed.
• Communicates changes to the plan of care to patient/family and any internal/external stakeholders involved.
• Complies with infection control and exposure control policies.
• Maintains effective relationships and partnerships with community healthcare providers and external stakeholders impacting the CAPC population.
