Position Summary

The Nurse Practitioner will be a trusted team member providing care to our patients and a trusted partner to the clinical team, center staff, and the Executive Leadership team.
Duties and Responsibilities:
Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic evaluations.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
Diagnose and treat acute and chronic illnesses, injuries, and health conditions.
Order and interpret diagnostic tests, such as lab work, X-rays, and EKGs.
Prescribe medications and provide patient education on treatment options and preventive measures.
Monitor patient progress, adjust treatment plans as needed, and ensure continuity of care.
Maintain accurate and up-to-date electronic health records (EHRs).
Participate in health promotion and disease prevention initiatives.
Qualifications:
Master's degree in Nursing with specialization as a Nurse Practitioner.
Active licensure as a Nurse Practitioner in California.
Strong clinical skills and ability to work independently.
Excellent communication and interpersonal skills.
