Job description
VCU Health is seeking an RN Care Coordinator to work within a multidisciplinary team to ensure high-quality, efficient transitions from acute care through post-discharge while maintaining safety and cost-effectiveness. The role requires active collaboration with physicians, social workers, and payers.
Required is Virginia RN licensure (or eligibility) and at least 3 years of acute care experience; a Bachelor's in Nursing or related field is required, with a Master's preferred.
