Job description
MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP to reshape the future of post-acute care. You will care for patients with acute and chronic diseases, working with a multidisciplinary team to deliver high-quality care.
The ideal candidate is a board-certified NP with 3–5 years in cardiology post-acute settings and at least 1 year in post-acute/long-term care. Expect autonomy, a supportive team, and opportunities for growth.
