Job description
Northwestern Medicine's general cardiology team is seeking a skilled nurse to support a complex patient population, collaborating with cardiologists, APPs and multiple departments to provide seamless care. The nurse will implement evidence-based interventions tailored to cardiology patients.
Responsibilities include patient assessments by phone or in person, initiating patient education, and coordinating care across teams.
