Job description
Advocate Health in Winston-Salem, NC seeks a Care Advocate Nurse (RN/LPN) to join the care-management team and collaborate with PCPs, hospital care managers and transitions teams to ensure high-quality, cost-effective care.
The role may be embedded in APP/AMG PCP or OB/specialist practices or work telephonically with a care team. You will manage referrals, develop care plans and coordinate with physicians to optimize outcomes.
