Description
Position: Care Coach
Department: Indiana Family Caregivers Care Management
Reports To: Care Coach Supervisor
Status: Part-time
Hours: Flexible to meet the needs of the Caregiver
Position Summary
The Care Coach position has been created to lend support to caregivers that are caring for a family member with complicated needs. As a care coach, you will go to the homes of our caregivers and help them find resources, offer support, and build good will. The goal of a Care Coach is to make 1 visit per quarter and build support for the caregiver. Encouraging self-care and helping the caregivers set goals for themselves. This position will support Cluster J (Hancock, Shelby, Rush, Fayette, Union, Franklin, Decatur, Monroe, Brown, Bartholomew, Lawrence, Jackson, Jennings, Ripley, Dearborn, Jefferson, Switzerland and Ohio counties).
Responsibilities/Job Duties
Make home visitsas required per state guidelines to ensure compliance.
Maintain records of visits
Communicate with the Care Coach Supervisor
Travel within the assigned territory to seecaregiversin their homes.
Coordinate visit times with the caregiver to ensure they are scheduled at mutually convenient times and commit to honoring scheduled appointments—cancelling only in true emergencies.
Document notes fromvisitinrequired apps on computer/ phone.
Attend monthly team meetings (virtually).
Perform other duties as assigned.
Required Qualifications
High school diploma or equivalent
Strong understanding of the emotional and physical demands placed on caregivers.
Experience working with medically complex populations.
Must be detail oriented.
Demonstrate good organizational skills and time management skills.
Must have the ability to travel on a weekly basis as this role is a field-based position.
Must have an active driver license and reliable transportationwith insuranceto drive throughout the day / territory.
Ability to effectively communicate with patientsandfamilies.
Strong critical thinking and problem-solving skills.
Musthave your own computer/ iPad or tablet and cell phone.
