CommonSpirit may ultimately pay more or less than the posted range as permitted by law.As our Care Coordination Assistant, you will be a vital caregiver and the central hub, assuming responsibility for patient care processes from newborn to geriatric for a designated time frame. You will directly contribute to safe, therapeutic patient care while serving as the primary receptionist and secretary. Every day you will support safe patient care through multidisciplinary processes and coordinating unit activities. You will efficiently manage information flow and assist healthcare team members in delivering care across all patient age groups. To be successful in this role, you will demonstrate strong organizational and communication skills, a proactive approach to managing day-to-day operations, and a commitment to providing safe, therapeutic care while working effectively with the entire healthcare team.Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.Manage timely post-acute care referrals, to assist with length of stay management and mitigation of denials.Communicate and collaborate with interdisciplinary teams, patients, and family to facilitate care progression.Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.Deliver required patient notifications and letters, and obtain patient or family signatures on required documentation as needed to meet regulatory compliance.Manage transportation referrals as requested.Required:Associates Other Associate (two-year) Degree in Business/Health-Related field, upon hire orHigh School Graduate General Studies, upon hire orHigh School GED General Studies, upon hire andPreferred:At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting. andBachelors Other or BSW, upon hire andExperience supporting clinicians in demanding and fast paced patient care environments.Licensed Practical Nurse: WA, upon hire orCertified Clinical Medical Assistant, upon hire orCertified Medical Assistant AAMA, upon hire orCertified Medical Assistant NHA, upon hire orNational Certified Medical Assistant NCCT, upon hire orNational Registered/Certified Medical Assistant NAHP, upon hire orRegistered Medical Assistant AMT, upon hire orRegistered Medical Assistant, upon hire orCert Med Asst - MMCI, upon hire orCertified Nursing Assistant: WA, upon hire orEmergency Medical Technician: WA, upon hireVirginia Mason Franciscan Health has a rich history of providing exceptional healthcare, dating back to 1891.
