Community Medical Centers seeks the System Director of Care Coordination to join the system leadership team, overseeing Social Services and Case Management. You will direct, coordinate, and evaluate care delivery across the continuum, using data trends to meet goals for length of stay, physician and patient satisfaction, throughput, and discharge planning.
You will mentor facility leaders and drive performance improvements.
