Job description
Southern Tier Connect in New York State seeks a Care Coordination Manager to lead comprehensive care coordination for individuals with I/DD. You will oversee a caseload, develop Life Plans, and work with families and advocates to support independence and community integration.
The role is hybrid, with in-office, field, and remote work options; requires NP/RN experience and strong documentation, HIPAA and IT security adherence, and flex scheduling to meet family needs.
