Job description
Rochester Regional Health is seeking a Care Facilitator in the Case Management department at Canton-Potsdam Hospital. This role is 40 hours per week, with a schedule from 8:00 am to 4:30 pm and one weekend every four weeks; travel to other sites is expected.
The position focuses on utilization management, discharge planning, care coordination, and referral to higher levels of care, ensuring efficient resource use and optimal patient outcomes while delivering superior customer service.
