Job description
The University of Kansas Health System is seeking an Ambulatory Social Work Case Manager – Transitions of Care for a part-time role at the Medical Pavilion in Kansas City, KS. You will assess psychosocial needs, coordinate services, and support patients and families through transition planning.
Responsibilities include collaborating with the care team, documenting findings promptly, and advocating for resources. Prior experience in health care and licensure as an LCSW/LMSW is preferred.
