nThe Care Coordination Specialist performs selected services and functions related Transitions of Care & Utilization Management, including insurance authorization, payer communications, Appeals and Denial information, and scheduling.

nEducation, Knowledge, Skills and Abilities Required:

nnnHigh School diploma, general equivalency diploma (GED), and/or GED equivalent programs.

n

nnExperience in the healthcare industry.

n

nnEffective written, oral, and interpersonal communications skills.

n

nnProficient computer, word-processing and Excel spreadsheet skills.

n

nnAbility to learn new computer systems, i.e.: EPIC, INDICIA.

n

nnEducated in the unique clinical and discharge needs of the patients.

n

nnStrong time management and priority-setting skills.

n

nnAbility to work independently and as a team member.

n

nnSelf-directed with the ability to take the initiative to solve problems.

n

n nEducation, Knowledge, Skills and Abilities Preferred:

nnA minimum of 3 years recent experience in a hospital or medical setting.

n nSome jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
