Job description
Prime Care Coordination seeks a Director of Care Coordination to lead the design and development of all care coordination services for the Care Coordination Organization/Health Home (CCO/HH). You will align strategy with PCC’s long-term vision and oversee day-to-day operations and staff in Upstate NY, ensuring compliance with OPWDD/DOH and cross-system integration.
