Job description
MidState Medical Center seeks an RN Care Coordinator to support safe, efficient, patient-centered care. In this role, you’ll coordinate transitions of care, develop and update care plans, and collaborate closely with physicians, nurses, social workers, and community partners. Responsibilities include discharge planning, patient and family education, utilization review, and removing barriers to timely care. You will track outcomes, document accurately in the EMR, support quality and safety initiatives, and help ensure patients understand their treatment and follow-up across the continuum of care.
