Care Management Assistant OpportunityLogan Health Medical Center is looking for a Care Management Assistant in Kalispell, MT! This newly created position will provide administrative and logistical support to our Case Management team across medical units such as Acute Care, Surgical, Progressive Care, and ICU.As a Care Management Assistant here's some of what you'll do:Assist with discharge planning and connect patients with identified resourcesSchedule follow-up appointments and coordinate next steps in careCoordinate durable medical equipment (DME) needsAssist with arranging patient transportationComplete and coordinate required Medicare (CMS) formsCollaborate with healthcare providers and other stakeholders to support timely and appropriate transitions of careHelp support the physical, psychosocial, educational, and spiritual needs of patients and familiesIdentify and escalate patient needs, barriers, and concerns to the appropriate care team membersWhat you bring:We are looking for a compassionate, organized, and detail-oriented professional with strong customer service and communication skills. This person should be a strong team player who can manage multiple priorities and follow through in a fast-paced healthcare environment.What we offer you:Affordable health benefitsTuition Reimbursement ProgramEmployer-paid life insuranceMatching 401(k)Logan Health Fitness Center Membership Discountand more!Don't miss this opportunity to grow your career in the healthcare field and gain some valuable inpatient healthcare experience!Qualifications:Related coursework beyond high school or experience in a complex administrative support position required.Minimum of one (1) year experience in a customer service-focused position required.Healthcare work experience preferred.Possess knowledge and understanding of medical terminology and medical coding preferred.Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.Commitment to working in a team environment and maintaining confidentiality as needed.Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.Job Specific Duties:Schedules and coordinates requested follow-up appointments for patients being discharged based on direction from the care management team.Orders and coordinates delivery of durable medical equipment as requested by the care management team.Assists patients with identified barriers to discharge follow-up, including transportation, appointment location, appointment availability, and scheduling. Escalates concerns or barriers to the appropriate care team member.Assists in identifying patients who may need follow-up care by reviewing medical records and communicating with care management team members, providers, and nursing staff.Assists with arranging resources and services identified by the care management team to support timely and appropriate discharge.Delivers CMS forms, including applicable Medicare notices, to patients in a safe, timely, and effective manner and communicates questions or concerns to the appropriate care team member.Communicates with healthcare providers, payers, physicians, and community-based services as directed by the care management team to assist with coordinating identified patient needs and services.Completes timely and accurate documentation in the medical record in accordance with departmental documentation standards to facilitate communication with care team members. Ensures documentation is completed in compliance with applicable guidelines and billing and reimbursement standards.The above essential functions are representative of major duties of positions in this job classification. Not all of the duties may be assigned to a position.Maintains regular and consistent attendance as scheduled by department leadership.Shift:Day Shift - 8 Hours (United States of America)Schedule: Full-time, 40 hours per week, 8-hour day shifts, Monday - Friday. No weekends!Logan Health operates 24 hours per day, seven days per week.
