Job description
Mission Health in Asheville, NC seeks a Care Manager Registered Nurse to coordinate and advocate for patients across the care continuum. In this role, you will perform comprehensive assessments, develop and implement individualized care plans, facilitate transitions of care, and collaborate closely with physicians, interdisciplinary teams, patients, and families. You will monitor outcomes, manage utilization, and ensure adherence to evidence-based guidelines while promoting patient education and self-management in a patient-centered, collaborative environment that supports professional growth and advancement.
