Job description
NYU Langone Hospital—Long Island in Mineola, NY is seeking a Nurse, Care Management - Full-Time, Days. The role coordinates post-acute services and ensures smooth transitions to the most appropriate setting.
You will work with the care team to optimize patient care and reimbursement. Responsibilities include performing utilization reviews, developing discharge plans with patient input, coordinating with physicians and the interdisciplinary team, and documenting in Allscripts.
