Job description
Molina Healthcare is seeking a Care Manager to support care coordination across the care team in Fort Lauderdale, FL. You will complete member assessments, develop and monitor care plans, and conduct visits as needed to drive outcomes and cost-effective care.
Qualified candidates have 2+ years in health care, strong communication, and the ability to manage multiple cases. Travel up to 40% may be required; a valid driver's license is needed where applicable.
