Job description
Molina Healthcare in Wisconsin is seeking a Care Coordinator with LTSS expertise to support member care across the continuum. You will coordinate services, develop and monitor care plans, and work with a multi-disciplinary team to improve member outcomes.
Responsibilities include comprehensive assessments, waiver enrollment processes, and collaboration with caregivers, physicians, and ICT. Travel up to 40% may be required; strong communication and independent work are essential.
