About This Role
The Care Manager Registered Nurse will collaborate with members, providers, and community organizations to address the health care and social needs of dual eligible populations. This role is ideal for experienced nurses who are passionate about improving patient care and outcomes.
Highlights
Pay: $66,575 - $142,576/year
Location: Aurora, IL
Residency: Must reside in the state of Illinois
Travel: Up to 75% travel required in DuPage, Kane, DeKalb, Kendall, and surrounding areas
Schedule: Full-time
What You'll Do
Utilize critical thinking and judgment to inform the case management process.
Facilitate appropriate healthcare outcomes for members.
Assist with appointment scheduling and accessing benefits.
Collaborate with members and community organizations to address health care needs.
Use care management tools and resources effectively.
Work independently and utilize virtual collaboration tools.
Demonstrate proficiency with standard corporate software applications.
Requirements
Education:
Associate's degree required, bachelor's preferred
Experience:
Minimum 3-5 years clinical practical experience
Preferred 2-3 years in Care Management, discharge planning, and/or home health care coordination
Licenses & Certifications:
Active and unencumbered Registered Nurse license in the state of Illinois
Certified Case Manager preferred
