About This Role
The Care Manager Registered Nurse - Field position at CVS Health involves collaborating with members, providers, and community organizations to address the healthcare and social needs of dual eligible populations. This role is ideal for experienced nurses who are independent thinkers and possess strong analytical and communication skills.

Pay: $66,575 - $142,576/year
Location: Moline, IL
Schedule: Full-time
Travel: Up to 75% travel required
What You'll Do
Utilize critical thinking and judgment to inform the case management process.
Facilitate appropriate healthcare outcomes for members by assisting with appointment scheduling.
Identify and assist members with accessing benefits and education through care management tools.
Collaborate with teams virtually using various tools.
Demonstrate proficiency with standard corporate software applications.
Maintain effective communication with members and providers.
Work independently and manage multiple systems and applications.
Resolve common technical issues independently.
Requirements
**Education:**
Associate's degree required, bachelor's preferred
**Experience:**
Minimum 3-5 years clinical practical experience
2-3 years in Care Management, discharge planning, and/or home health care coordination preferred
**Licenses & Certifications:**
Active and unencumbered Registered Nurse license in the state of Illinois required
Certified Case Manager preferred
