About This Role
The Care Manager Registered Nurse - Field position at CVS Health involves collaborating with members, providers, and community organizations to address healthcare and social needs for dual eligible populations. This role is ideal for experienced nurses who are passionate about improving patient care and outcomes.

Pay: $66,575 - $142,576/year
Location: Cicero, IL
Schedule: Full-time
Travel: Up to 75% travel required
What You'll Do
Utilize critical thinking and judgment to inform the case management process.
Facilitate appropriate healthcare outcomes for members.
Assist with appointment scheduling and accessing benefits.
Collaborate with teams virtually using care management tools.
Demonstrate proficiency with standard corporate software applications.
Communicate effectively with members and providers.
Work independently and manage multiple systems.
Resolve common technical issues independently.
Requirements
Education: Associate's degree required, bachelor's preferred
Experience: Minimum 3-5 years clinical practical experience
Preferred 2-3 years in Care Management, discharge planning, and/or home health care coordination
Licenses & Certifications: Active and unencumbered Registered Nurse license in the state of Illinois
Certified Case Manager preferred
