Job description
Providence Tarzana Medical Center is seeking a Care Manager RN in Tarzana, CA. This full-time role features 8-hour, variable shifts and a focus on coordinated care across disciplines to support transitions and optimize patient outcomes.
The position requires a California RN license and a Bachelor’s degree in Nursing (or agreement to obtain one within 3 years). Experience in Case Management or TIP is valued, with a strong emphasis on collaboration and patient advocacy.
