About This Role
The Care Manager - RN in the Medicine Units at NewYork-Presbyterian is responsible for managing complex cases and ensuring continuity of care for patients. This role is ideal for a driven and tenacious individual who can lead nursing practice in a respected university medical center.

Pay: $134,900 - $168,000/year

Schedule: Full-Time, Day shift
What You'll Do
Manage complex cases and serve as a liaison between acute care and critical care teams.
Coordinate efforts of social workers, feeding specialists, PTs, OTs, cardiologists, intensivists, and neonatologists.
Direct all aspects of inpatient care, ensuring optimal length of stay.
Anticipate outpatient service needs, setting appointments and consultations prior to discharge.
Requirements
Education
Bachelor's degree in Nursing (BSN) required
Experience
3 to 5 years of healthcare experience in an acute care setting
2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting preferred
1 year of leadership experience in a healthcare setting preferred
Licenses & Certifications
Current NYS Registered Nurse (RN) licensure required
