IMC is currently hiringaRegistered Nurse - Care Manager , who under the generalsupervision of the Unit Director and with the authority of the Chief NursingOfficer, will apply nursing process (assessment, planning, intervention andevaluation) in caring for patients. Effectively interact with patient,significant others, and other heath team members. Demonstrates knowledgeand skills necessary to provide care based on physical, psycho/social,education, safety and related criteria for the newborn, pediatric, adolescent,adult and geriatric population.
EDUCATION
Graduate of state approved School of Nursing.
LICENSURE/CERTIFICATIONS
Current licenses as a RN in the state of Louisiana.
TRAINING/EXPERIENCE
Previous experience preferred, but not required.
RESPONSIBILITIES
Be available by telephone during workinghours
Act as liason between patients,physicians, and referral sources.
Be knowledgeable of all assignedpatients.
Conduct on-going clinical reviews toensure appropriate level of care and necessity of continued stay.
Reports clinical reviews to third partypayer sources as required/requested.
Post authorizations into computer asneeded.
Make appropriate referrals as orderedper MD.
Ensures that the most effective andlevel appropriate discharge plan is in place for all assigned patients to promotesafety in the post-acute setting.
Actively involved in interdisciplinaryrounding meetings.
Complies with all complianceexpectations.
Knowledgeable of all CMS Mandated Forms.
