Job description
Advocate Health is seeking an experienced LPN/RN care manager in North Carolina to coordinate complex patient care under the Advocate Care program. You will assess referrals, design and adjust care plans, and collaborate with physicians, nurses, and community partners to improve outcomes.
The role requires RN with BSN or LPN with extensive UM experience, strong communication, and EMR proficiency. It offers a 40-hour week, first shift, and a comprehensive benefits package.
