Job description
Fairview Skilled Nursing Home in New York seeks a compassionate RN to coordinate individualized resident care plans, collaborate with the interdisciplinary team, and ensure high-quality, resident-centered care.
The Care Plan Nurse will develop, update, and maintain care plans, lead interdisciplinary care plan meetings with nursing, therapy, dietary, social work, and physicians, and ensure compliance with CMS and New York regulations. Strong communication and organizational skills are essential.
