When you join our team, you become part of a compassionate, purpose-driven organization committed to making a real difference.What We Offer:A collaborative and supportive work environmentA meaningful opportunity to positively impact lives every dayCompetitive compensation packagesTuition reimbursement for full-time employeesFree continuing education opportunities for all team membersClear paths for career growth and advancementComprehensive medical, dental, and vision insurance401(K) with employer matchGenerous paid time off, including holidays and family/pet bereavementPet insurance for your furry family membersAs the Care Team Supervisor, you'll contribute to our success in the following ways:Coordinate and schedule homemaking, personal care, and home health aide services in a timely manner, including coverage for planned and unplanned absencesMaintain accurate client service records, plans of care, caregiver schedules, and required documentationReview and manage contract-specific documentation and ensure all required authorizations align with scheduled servicesEducate clients on services, policies, and Federal/State regulations to ensure understanding and complianceSupport branch growth initiatives and operational planning effortsReview reports and monitor systems to ensure accuracy in billing, payroll, utilization, and service deliveryProcess and manage employee timesheets and collaborate with payroll and timekeeping teams as neededEnsure ongoing compliance with all State, Federal, licensure, contract, and regulatory requirementsProvide guidance, coaching, support, and performance counseling to caregivers to promote engagement and quality careEnsure caregiver compliance with required trainings, certifications, medical requirements, health assessments, and in-servicesConduct caregiver orientations and maintain complete caregiver and client documentation recordsMonitor daily client service delivery requirements to ensure adequate caregiver availability and shift coverageReview open shifts, utilization reports, and staffing needs daily to maintain continuity of careSubmit employee incident, allegation, and workers' compensation information to HR in a timely mannerCommunicate professionally and effectively with caregivers, clients, case managers, supervisors, and leadership teamsRepresent Elara Caring in a professional and positive manner within the community and through participation in local organizations and initiativesRemain flexible and available to support direct care services as neededMay oversee and provide guidance to Chinese-speaking supervisors to ensure alignment with branch and organizational objectivesSupport Elara Caring's mission, values, and commitment to delivering exceptional patient-centered care every dayPerform additional duties and projects as assigned to support team and organizational successWhat is Required?High School Diploma or GED requiredAssociate's or Bachelor's degree preferredExperience working with elderly or disabled populations preferredFive (5) years of experience in a fast-paced office or healthcare environment preferredWorking knowledge of Medicare/Medicaid and home care operations preferredKnowledge of kidney-related diagnoses, including dialysis treatments and post-dialysis care needs is highly preferredStrong computer proficiency and documentation skillsExcellent organizational and multitasking abilitiesProfessional verbal and written communication skillsStrong leadership, coaching, and motivational skillsAbility to thrive in a fast-paced, high-volume environmentBilingual fluency (Spanish, Mandarin/Cantonese, Russian, or Portuguese) may be required based on locationReliable personal transportation and ability to travel locally and occasionally to support audits or operational needsYou will report to Care Team Manager, Lead, Branch Director, or Regional Branch Director.This is not a comprehensive list of all job responsibilities; a full job description will be provided.We are committed to equal employment opportunity regardless of race, color, ancestry, religion, sex, national origin, sexual orientation, age, citizenship, marital status, disability, gender, gender identity or expression, or veteran status.
