nWork hours: Full time; Monday - Friday; 8:30 - 5:00p

nnn H.S. Diploma or Equivalent requirednn
nn Three years' relevant experience, preferably in a healthcare or service environment requirednn
nn Patient Care/ Customer Service / Healthcare experience strongly preferrednn
n nWhat You Will Do
nnn Responsible for coordinating the clerical and administrative processes for entity care transitions managementnn
nn Responsible for managing deparmental mail box, phone lines and/or faxnn
nn Supports transition planning activities as needed under the direction of care transition managersnn
nn Calls vendors, physician offices, clinics, etcnn
nn Faxes (electronic and traditional) documents as needednn
nn Makes copies or gathers other data as needednn
nn Collect and enter data as needednn
nn Completes other tasks and projects assignednn
