Job description
Nebraska Medicine is seeking a Care Transitions Nurse in Omaha for a full-time position. You will coordinate patient-centered discharge plans, work with an interdisciplinary team, and ensure seamless transitions of care from inpatient to post-acute settings.
The role requires a BSN and a Nebraska (or compact) RN license, with strong communication and critical thinking skills to guide patients and families through complex care journeys.
