Job description
Providence at Home with Compassus is seeking a Care Partner to coordinate safe, patient-centered transitions of care for hospitalized patients and evaluate options for post-acute home-based services. You will support timely discharge planning in collaboration with physicians, case management, patients, families, and post-acute providers.
This in-person role emphasizes bedside engagement, interdisciplinary collaboration, and active participation in discharge workflows.
