Job description
Intandem in New York State seeks a Plan Specialist - SEMP to support adults with disabilities by developing person-centered plans and coordinating activities, documentation and reporting of progress toward goals.
You will collaborate with the ETP supervisor, engage community partners, provide job coaching for OPWDD and ACCES VR participants, and help prepare Discovery paperwork to identify suitable candidates for the program.
