Required Document:Social Security CardDriver's LicenseAuto Insurance (if applicable)Health Certificate (physical exam and TB results within 6 months)Training Certificates in:HHA/ CNA LicenseBLS/CPR CardAlzheimer's Disease (2-hour course) Approved by the Department Of Elder AffairsAssistance with Self-Administered Medications (2-hour course)Domestic Violence (2-hour case)HIPAA (2-hour course)HIV/AIDS (4-hour course)OSHA/Bloodborne Pathogens (2-hour course)Background Screening Requirement: This position will require background screening through the Clearinghouse.
