We are committed to enhancing the quality of life for our residents and pride ourselves on fostering a warm and supportive environment where personalized care is tailored to meet specific needs.Job SummaryWorking as a Caregiver in our Community Based Care Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with residents, their families, and care team to provide personalized care that meets each individual's unique needs and preferences.ResponsibilitiesProvide assistance with activities of daily living (ADLs).Monitor and report any changes in residents physical or mental health.Assists with serving resident meals.Provide companionship, emotional support, and meaningful engagement to residents.Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.Maintain resident confidentiality.Encourage clients to participate in activities that promote physical, mental, and emotional well-being.Support clients in maintaining independence in their daily routines and decision-making.Ensure a safe environment by identifying and addressing potential hazards in community setting.Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.Present for staff meetings and training.Other duties as assigned.RequirementsMust be 18 years of age, N AC/CNA or NAR or HCA acquired within 120 days of hire, High school diploma or equivalent required. Previous experience in caregiving, home care, or community-based care is preferred. Ability to work independently and as part of a team. Patience, empathy, and a genuine desire to help others. Good organizational and time management skills. Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility. Must be able to stand, walk, and reach for extended periods. Ability to pass a background check.
