Home Care ProviderRequirements documents: Social Security (Card), Driver's License / ID, Employment Authorization/Permanent Resident/USA Passport, Auto Insurance, Background screen, Direct Deposit, Health Certificate (less than 6 months), TB results (less than 6 months), BLS/CPR Card, HHA (75 hs) or CNA License, Alzheimer's (2hs) Approved by the "Department of Elder Affairs", HIV/AIDS (2hs), Assistance with Self Administration of Medications (2hs), Domestic Violence (2hs), OSHA and Infection Control (2hs), HIPAA (2hs)Background Screening Requirement: This position will require background screening through the Clearinghouse.
