nThe Case Management Assistant responsibilities include:
nnnAssists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
n
nnResponsible for assisting with distributing regulatory documents to identified patients
n
nnEducating patients about the appeal process related to their discharge
n
nnPerforming multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
n
nnRequires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives
n

nnWeekend Requirement: Occasional
