About the Opportunity
A confidential healthcare organization is seeking an experienced Case Management Director to oversee clinical case management and care coordination functions.
This position is responsible for providing leadership, supporting clinical operations, coordinating patient services, and helping ensure appropriate care throughout the patient experience. The ideal candidate will have significant nursing experience along with a strong background in case management and team leadership.
Responsibilities
• Provide leadership and direction to case management personnel.
• Oversee patient care coordination and transition planning.
• Review clinical information and documentation to support appropriate patient services.
• Work with physicians, clinical teams, patients, families, and external partners.
• Support utilization and authorization processes.
• Assist with complex cases, coverage concerns, and care coordination challenges.
• Monitor department processes and identify opportunities for improvement.
• Coach and support staff while promoting effective teamwork and communication.
• Maintain compliance with applicable healthcare regulations and organizational policies.
Qualifications
• Active Nevada RN license.
• Graduation from an accredited nursing program.
• Current BLS certification.
• At least 5 years of nursing experience.
• Previous leadership or supervisory experience.
• Strong case management or care coordination background.
• Acute care experience preferred.
• Bachelor's degree in Nursing preferred.
• CCM or ACM certification is a plus.
