Job description
Community hospital part of a system looking to bring on Case Management Director! Bonus Incentive Program, Sign On Bonus and Full Relocation!
Overall responsibility for managing and coordinating department activities.
Ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the department's patient care and discharge planning processes.
Oversight into all aspects of daily hospital case management operations and is accountable for achieving established outcomes.
Reports to Division Director of CM.
Overseeing 13 employees (1 lead CM, 1 CMA, 2 SW, 9 RN CM - 4 are PRN).
Qualifications:
Bachelor’s Degree
Master's degree in Nursing, Health Administration, or Business Administration, preferred
(RN) Registered Nurse, or (LCSW) Clinical Social Worker, (RN) Registered Nurse preferred
3 years’ experience in overall acute care Hospital Case Management, preferred
2+ years’ experience in case management leadership in an acute care setting MANDATORY
