n nOur Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment.Based out of Fort Pierce, FL, this position is responsible for supporting Dependency Case Managers with assigned tasks, transporting clients to and from court appointments, and medical appointments, coordinating and supervising family visitations, supervising children, and documentation of those appointments. Additional responsibilities of the position include:

nnnProvide transportation for dependent children and parents if needed to medical appointments, counseling, court, placements etc.

n

nnProvide long distance transportation (may result in overnight stay for employee)

n

nnSupervise children.

n

nnCoordinate visitations between case managers, foster parents, parents, children and caregivers.

n

nnTransport and supervise visitation between children, parents, caregivers, etc. (Majority of job)

n

nnDocument in electronic case file all contacts made as well as observations, interactions, behaviors of children, parents and caregivers.

n

nnAssist Dependency Case Managers in providing case management services (information, referral and linkages).

n

nnAssist Dependency Case Managers in obtaining records and documents needed for the delivery of services (e.g., education records, medical/dental records, vital statistics, etc.)

n

nnTestify in court hearings regarding observations/documentations.

n

nnEffective time management.

n

nnAttend staff meetings.

n

nnBe available to work after hours and/or on weekends as needed.

n

nnEffective verbal and written communication.

n

nnParticipate in the accreditation and re-accreditation processes.

n

nnPerform other duties as assigned.

nQualifications

nEducation:

nnHigh School diploma or GED equivalent required

n nExperience:

nnn1+ years related experience required (preferably working with children); 2+ years of experience working with children or families preferred

n

nnPrevious experience in a social service organization preferred

n

nnPrior experience installing children's car seats, types of seats, requirements, and expiration dates

n

n nOther Requirements:

nnnValid Driver's License in good standing required

n

nnClass B CDL License with passenger endorsement a plus

n

nCommunities Connected for Kids provides oversight and coordination of the child-welfare system in Indian River, Martin, Okeechobee and St. Lucie counties. At any given time, there are about 1,300 abused, abandoned or neglected children in our care. We are responsible for the safety and well-being of each one of them. We meet this responsibility through a mix of direct and contracted services. If you think about it in terms of building a house, we are the general contractor of the child-welfare system. The system is the house, we are the contractor, and our network of service providers are the subcontractors we pull together to make the house strong and ready to live in.

nCommunities Connected for Kids offers a full range of care for children, adolescents, young adults and family members in Okeechobee and the Treasure Coast. We offer our services through a wide variety of approaches including in-home parenting and prevention services, family preservation, case management, residential programs and extended foster care services for teenagers and young adults.

nIn the tradition of true partnership, Communities Connected for Kids works closely with several agencies that are funded through Medicaid, substance abuse/mental health or private funds to offer a continuum of treatment services otherwise unfunded by Community Based Care dollars. These partnerships help families strengthen their capacity to care for their children or help children heal from trauma they’ve experienced.
